To solve this problem you must apply the proccedure shown below:

 1. You have the following information given in the problem above:

 - The vertex of the parabola is (-3,-13) and the y intercept is -4.

 2. You have the the Standard form of the equation of the parabola is:

 y=a(x-h)^2+k

 Where the vertex is (h,k)

 3. Therefore, you have:

 -4=a(0-(-3))^2-13
 a=9/9
 a=1

 4. Finally, you have that the equation is:

 y=(x+3)^2-13
